Breast Implant Options

There are two main types of breast implant material; saline solution and silicone gel. Both types of implants are housed in a silicone shell specifically developed to resist punctures and ruptures. In addition to strength, there are also different finishes on the outside of the shells designed to augment the specific type of implant used.

Saline implants are composed of a sterile salt and water solution housed in a silicone shell. Silicone implants are composed of medical grade silicone housed in a silicone shell. The silicone can be semi cohesive or form stable, depending on the thickness of the silicone gel.

Saline Implants: Advantages and Disadvantages

One of the major advantages with saline implants is the size of the incision. Since the implant shell is inserted while deflated, the incision will be much smaller than in silicone gel implant surgery. A smaller incision makes for a very small scar, which over time becomes virtually invisible.

Another big advantage is that during surgery, the surgeon can vary the volume of fluid going into the shell so that the most ideal size is achieved for the patient. This ‘try it before you buy it’ option isn’t possible with silicone gel implants.

With a saline implant, the silicone shell is filled with a sterile saline solution. In the rare occasion that the silicone shell should rupture, or in the case of trauma to the chest, the saline solution that leaks out is harmlessly eliminated by the body.

Also, if the implant should rupture it will be immediately obvious as the breast deflates. In this way patients can visit their plastic surgeon to have the implant immediately removed and replaced.

Disadvantages to consider with saline implants

Saline implants look natural but may not feel as natural as silicone gel implants. In thin patients the silicone shell may be visible under the skin, particularly at the bottom of the breast. This will appear as wrinkling or rippling under the skin. Another possible disadvantage is that saline implants have a slightly higher incidence of capsular contracture than silicone implants. This is a condition caused by scar tissue that shrinks or contracts around the implant, causing it to become hard and malformed in extreme cases.

This isn’t only an issue in breast implant surgery. The body naturally creates a scar tissue capsule around any foreign object placed in the body whether it is a breast implant or an artificial hip. Since the breast lacks any hard structure, any amount of contraction of scar tissue is very evident.

Silicone Implants: Advantages and Disadvantages

There are actually two types of silicone material used in breast implants: semi cohesive (or high strength silicone gel) and form stable gel. Both types are composed of medical grade silicone. The only difference is in the consistency of the silicone.

Semi cohesive silicone is very thick and does not run or squirt. It is chemically attracted to itself so tends to return to the original shape where possible. For example, if there is a puncture or rupture in a silicone shell filled with this type of gel, it will squeeze out under pressure but will retract into the puncture once the pressure is removed. Form stable silicone gel is solid but spongy, similar to memory foam mattresses. Even if the shell is cut in half, the silicone stays in place. This type of silicone has been nicknamed the ‘gummy bear’ implant. It is formed into a teardrop shape with most of the fullness at the bottom as in the natural breast.

Advantages of silicone implants

Safety is the prime concern in any decision concerning implants. There has been a significant amount of press coverage concerning rupture of silicone implants and the fear of compromised health.
